  • Cost: $90.00  - $75  with proof that your dog was adopted from a shelter or rescue group or that you are raising a dog for an assistance organization.
  • Register: Download & complete registration form below.  Mail registration,  payment & copy of immunization records & adoption paper if you meet the criteria above.  Mailing address is on the top of the registration form.
